Orthodox and heretical methods for mesh adaptation

Abstract
Starting from the hierarchical method for error estimation, we show how
this can be implemented in a general mesh adaptation code. This would be
an orhodox mehod. It can then be generalized by approximating the
hierarchical estimate through higher order reinterpolation of the
results. This is heretical but we provide a partial justification.
We present two and three-dimensional numerical results
